Lutron Skylark SLV-600P-IV ivory slide dimmer. A high quality Commercial Specification Grade dimmer; don't confuse it with light duty residential models. Features include:

Rocker switch turns lights on to desired preset level
Slide control dimmer adjusts the light to suit any activity
Rated 450W 120VAC incandescent; rated 600VA magnetic low voltage - not suitable for electronic (solid state) low-voltage transformers
Single pole or 3-way installation when used with a 3-way switch (sold separately)
Power failure recovery ensures retention of last setting before power interruption
Fluid slide movement allows fine adjustment of light level with minimal effort
